Chronic Disease Coordination

The Chronic Disease Coordinators primary role is to work with health care services, individual health care providers, the Pilbara Aboriginal Health Forum and aboriginal communities to:

  • Develop a Regional Plan to improve Diabetes and Kidney Disease awareness and management
  • Facilitate better coordination of the prevention, early detection and management of chronic disease with a particular focus on diabetes and kidney disease
  • Develop strong relationships and networks across the region to promote and improve effective communication between all parties involved in chronic disease awareness and management
  • Identify new chronic disease initiatives and funding opportunities and provide implementation support where chronic disease programs are deemed suitable
